• a) Due to the expiry date

• Have a minimum of 6 (six) months prior the expiry date of the Identity Document or the Birth Certificate, and provided that the issuance of the said document was done before 2002

• b) Due to poor conservation

• The applicant should present the original Identity Document

• c) For endorsement

• The applicant should present the original of his/her full Birth Registration Certificate or his/her full Birth Certificate

• d) For loss of document

• Copy of the Identity Document (if available) accompanied by the Original Declaration of Identification

• e) Due to theft incident

• Copy of the Identity Document (if available) accompanied by the Police Report

Documents and the proof of payment must be submitted in physical format to the Consular Section of the Embassy.
