Transcript of Death Certificate

• Death Certificate issued by the local Institution, duly authenticated and officially translated

• Officially translated copy of the original Medical Report of the cause of death,

• Officially translated copy and original of the burial order

• Copy and original of the Identity Document

• Copy and original Passport

• Copy and original of the Consular Card

• Copy and original of the Identity Document of the declarant or family members

• Copy and original Passport of the applicant or family member

Documents and the proof of payment must be submitted in physical format to the Consular Section of the Embassy.
